We are always welcoming new patients to our practice
If you are an existing patient and would like to update your personal/insurance information or medical history please start a new Health History form and fill out the required portion and any new or changed information.

Scheduling
We try to accommodate the busy schedules of our familes whenever possible, but it is our policy to appoint pre-school children and those in the primary grades during the morning hours. Official school excusal forms will be provided if necessary. Late afternoon appointments and Saturday hours are reserved for children in the secondary grades, high schoolers and for the convenience of working parents. Emergencies will always receive priority.
Should it be necessary for you to miss an appointment, kindly notify us at least 48 hours in advance so the time can be allotted to other children waiting for an appointment. Failure to do so will result in a charge for lost time, except in the case of an unforeseen problem.
Every effort is made to maintain our appointment schedule. But working with children comes with an inherent unpredictability. So, please be patient and enjoy our people friendly waiting room.
Financial Policy
Our fees are based on the treatment necessary and time involved and may vary according to the intricacies of each case. The doctor's plan of treatment including the fees will be presented to you and upon approval, treatment will commence.
As you are aware, payments for services are due at each treatment visit. Any outstanding balance must be paid in full upon completion of treatment. For your convenience we accept Visa, MasterCard and Discover. Monthly billing is not part of our usual routine.
In the case of dental insurance, please remember that services are rendered for and charged to the patient and not your insurance company. We are not responsible for the limitations of your insurance company. Insurance is intended to be of assistance in paying the doctor. There is no connection between our fees and what your insurance company is willing to pay. The limitations of your insurance coverage is yours... not ours.
For the sake of maintaining our doctor - patient relationship, we require that you pay our fee directly to us and subsequently we will complete any necessary insurance claim forms, at no charge, to assure maximum reimbursement to you by your insurance company. If your insurance company does not pay for treatment within 60 days you must pay the balance and it is then your responsibility to call the insurance company to inquire as to why they have not paid for your child's treatment.
As conscientious pediatric dentists, it is our obligation to provide the finest care possible for your children. We therefore do not allow insurance companies or financial overtones to compromise the ideal treatment we offer to every child. Your cooperation is greatly appreciated.
